Reta dan Santi bersahabat sejak kecil. Mereka sempat berpisah selama beberapa tahun dan bertemu kembali saat kuliah. Reta jatuh cinta pada teman kuliah mereka yang juga kekasih Santi bernama Daniel (Dimas Aditya). Mengetahui hal itu, Santi menyembunyikan hubungannya dengan Daniel, terlebih ketika Santi tahu kalau Reta mengidap penyakit kanker otak dan divonis hidupnya tidak lama lagi.

Santi ingin membuat Reta senang di akhir hidupnya, akhirnya ia mempunyai ide untuk merelakan kekasihnya Daniel agar berpacaran dengan Reta selama tiga bulan. Waktu terus berlalu, dan bulan ini Reta dan Daniel sudah merayakan hari jadian mereka yang ke 1 (satu) tahun. Santi gelisah hatinya, karena ternyata Reta masih tetap hidup dan malah makin terlihat sehat. Ternyata Reta membohongi Santi selama ini tentang penyakitnya yang hanya rekayasa Reta. Daniel sangat menyesali perbuatan Santi, tapi ia membantu Santi menyingkirkan mayat Reta. Di perjalanan, mereka mengalami kecelakaan dan Daniel tewas. Santi yang luka parah dilarikan ke sebuah rumah sakit oleh sepupunya, Moniq (Monique Henry). Ketika Moniq sedang menemani Santi di saat-saat komanya, tiba-tiba saja keanehan terjadi, Moniq merasa diteror oleh sesuatu yang sangat menyeramkan. Moniq akhirnya mengetahui seluruh permainan cinta dari buku harian Santi. Moniq menyesalkan Santi karena sudah melakukan hal sebodoh itu. Tiba-tiba Santi bangkit dan mencoba menyerang Moniq.
